Endangered and Threatened Species Permit Application (1700-001)
This permit is required to take, transport, possess, or sell any animal or plant species or parts thereof included on the Wisconsin Endangered and Threatened Species List. View Form [PDF 7KB]
Natural Heritage Inventory (NHI) Endangered Resources Review Request (1700-047)
It is often advantageous for an applicant to gather endangered resources information for a proposed project early in the planning process, even if the project will be reviewed later in the process by other DNR staff. To find out if a proposed project may impact rare species or other sensitive resources, an applicant can request an Endangered Resources Review of the proposed project. This Review provides a letter to the applicant listing rare species, high-quality natural communities, and significant natural features found in or near the proposed project area. The letter will also explain whether additional steps need to be followed to comply with the Wisconsin Endangered Species Law. You may want to include a copy of this letter with any applications for permits or formal approvals for the project that you submit to other DNR programs. Response time is generally 6 to 8 weeks, but varies depending upon workload. The fee for an Endangered Resources Review is $20 per hour, with a minimum fee of $60. Projects will be invoiced upon completion. You may fill this form out on your computer, print, and mail out. However, you can not save the form, as any filled-in fields will be lost. Wisconsin Colonial Waterbird Online Survey (1700-058)
This form was designed to streamline the data submission process for professional wildlife biologists and experienced volunteer cooperators who have conducted or are conducting colonial waterbird surveys during the course of spring and summer outdoor activities. View Form
The following field report forms are for use by field observers when recording observations of rare plants or animals. The forms provide standardized information that will help us to enter the data and to interpret the observation for that sighting (and in the larger context of Wisconsin). The form should be filled out as completely as possible and sent to the address listed at the bottom of each form. The Natural Heritage Program cannot accept data derived from trespass. Gain landowner's permission before entering private property.
